If you are unable to keep your scheduled appointment, we ask that you give us a minimum of 24 hours notice. If sufficient notice is not given, a charge may be assessed.
For your convenience, we are happy to file your primary PPO dental insurance; however, please understand that our offices only participate with Delta Dental of the USA Premiere Network and selected dental plans through United Concordia. Please check with your insurance company if you are unsure of your plan type or coverage. If you have secondary coverage, we will be happy to supply you with a print out of your services so that you are able to self submit.
